National Diabetes Prevention Program

Who? If you are over 18 and have other risk factors for type 2 diabetes, this evidence-based program run by the Centers for Disease Control and Prevention (CDC) is highly effective at preventing a diagnosis of diabetes. If you have been told that your blood sugar is higher than normal (prediabetes) or you get a 5 or above on the risk test, this lifestyle program is for you.

What will I do? Set a weight loss goal and an activity goal. Determine baselines and progress through the CDC Prevent T2 sessions (26) with your DPP team to set action plans and practice behaviors that can edge your blood glucose to normal levels and prevent diabetes. Use behaviors to eat well, move more and manage stress. Stay accountable.
When? 26 1-hour sessions over 1 year, meet weekly 1st 6 months and then monthly for the 2nd 6 months
